Make a Referral to LimbTech

Our team will attempt to begin the scheduling process within 24-48 hours of receiving a referral. 

If you are referring from a doctor's office, we will coordinate with a local DME supply company to schedule the appointment.

We accept many major medical insurances. Please inquire if the patient has a less-common insurance. For patients with insurances that we are out of network with or patients receiving home health services who have Medicare or Humana Gold plans, we offer a self-pay rate of $100.

We require a signed OT Rx in order to schedule a patient. We have attached our referral form below to utilize, or you may use your own. 

Please include the patient's insurance policy on the referral and all demographics.
