Payment Information
LimbTech is currently licensed and credentialed in North Carolina, South Carolina, Florida, Georiga, Kentucky, Indiana, California, Virginia, Texas, Oregon and Tennessee. Payments can be made through insurance. We also have the option of a cash rate for evaluations.
NOTE: We must have an OT referral from a medical provider before evaluation is completed.

We do offer a self-pay option of $100 to complete your occupational therapy evaluation for out-of-network and unique circumstances. With this option, it does ensure your LMN will be finalized and sent out the same day as your evaluation.
